PURPOSE:
    - The Pain Patient Profile (P-3) is a questionnaire developed to
      identify patients who are having psychological problems
      associated with their physical pain and dysfunction. The P-3
      consists of 44 multiple-choice questions, each having three
      response options.  The instrument can be used as a pre-treatment 
      screening device, as a means of tracking and planning treatment, 
      or as means of determining if mental health consultation is
      needed;
